I'm thinking that if synt is routinely given as part of third stage as part of active management, then this may be deemed to be the reason why...ie, the need to actively manage (early cord clamping and CCT) the birth of the placenta in water. If however synt is only used in the event of bleeding after a physiological/expectant management of third stage in water...I would probbably want the woman out of the bath so as to be able to gain a more accurate assessment of her blood loss...
