I am in line with the other IP /MW comments on tears at home.
Complicated tears require expert suturing & that's what OBs are specialists at, dealing with abnormal events.
I have been suturing for 16 years & will do anything I feel I can do well.If I felt I couldn't do a good job ,I'd refer.
I have not had to transfer anyone to hospital (no 3rd degree tears, thank god)  for suturing but wouldn't hesitate if need be.
Have once T/F a mother (a fellow MW) to her GPs for sutures, he was OK with it, disruptive +++ for her though &  I wouldn't need to do that now as I am  probably more experienced than the GPs at suturing peris here.
Just my take on it !

I am curious about something (and I know it is fairly rare with homebirth) but for experienced midwives I assume you would stitch tears yourself even if they were 3rd or 4th degree, but I don't want to make assumptions so is that the case? Or would you transfer to hospital to have a doc stitch it,  or for  the extra pain relief so you could stitch it yourself? At what point?
