Yes item level auditing will track the permission changes .

skills and have money to spend on the problem.

You would need to apply the SPAuditMaskType.SecurityChange audit mask type.

Could it be that someone is changing the library level permissions to inherit,

OOTB site level auditing only seems to capture permission changes at the
library/list level.

Don't quote me on this but I seem to recall that the Item Level Auditing
solution is able to do this for list items as well.

Subject: Item Level Permissions Keep Resetting

Hi Guys

We have a document library which contains a bunch of spreadsheets which have
item level permissions applied to them. For some reason every now and then I
have found that the item level permissions seem to be removed. I have checked
the audit log to see who has changed the permissions on the library but
unfortunately there is no data available indicating that no permissions have
been changed

Any ideas on what might be causing it?

Regards,

Nathan
