It's not required by default.

If you have a look at:
12\TEMPLATE\FEATURES\DocumentLibrary\DocLib\schema.xml you'll notice the
ContentTypes node references content type: 0x0101
The content type definition in 12\TEMPLATE\FEATURES\ctypes\ctypeswss.xml for
0x0101 has the following Field defined for Title:

<FieldRef ID="{fa564e0f-0c70-4ab9-b863-0177e6ddd247}" Name="Title" *
Required="FALSE"* ShowInNewForm="FALSE" ShowInEditForm="TRUE"/> <!-- Title
-->
