Try using SharePoint Manager http://www.codeplex.com/spm (make sure you get the 
2007 version)

Great for checking event handlers are binding to the list or content type you 
are using

Yes.  I tried debugging it but it doesn't get called at all - even though it 
shows up as being bound to the Document Library.
Similar code and .xml file setup has worked fine with Custom Lists/Calendar 
Lists and Tasks Lists (depending what I set the ListTemplateId to) before.

Also my logs to the Event handler do not get logged so this is another reason 
why it looks like it doesn't get called whatsoever.

The SharePoint Logs are Verbose and show nothing useful in relation to this 
Event Handler.

The difference here (than the similar code for lists) is that I am attaching it 
to a Document Library ... but I have the correct ListTemplateId for document 
libraries - 101.  It activates without error through the Site Features.
It is scoped to a Web.

Not sure where else to go on this.
