Hi Francois,
It was missing.  Adding the sql.conf include to the radiusd.conf file
fixed the problem.  Thanks for you help tracking this down.
 
This was the original code in the radiusd.conf file:

        modules {
                $INCLUDE ${confdir}/modules/
                $INCLUDE eap.conf
        }
        

 
I modified it to: 
 

        modules {
                $INCLUDE ${confdir}/modules/
                $INCLUDE eap.conf
                $INCLUDE sql.conf
        }

Can you have a look in the radiusd.conf file, and look for :

You should have it right under:
