OK, first a big thank you to fhaynes of irc fame and James of Inverse!
The problem seems to be that the latest version of the Config::IniFiles perl
module available in the default Ubuntu 12.04-2 LTS repos does not work with
stock PF 4.0.1.
However installing the package from cpan or a later ubuntu release fixes the
issue.
The version available in the repos is:
libconfig-inifiles-perl 2.78-1
cpan has:
SHLOMIF/Config-IniFiles-2.82.tar.gz

You should update the perl module Config::IniFiles
One of the following packages
perl-Config-IniFiles for CentOS or
libconfig-inifiles-perl on debian

I am trying to add a user source in the admin GUI in 4.0.1 and am getting a
small red banner with the following error:
Error! _caseify not found at /usr/local/pf/lib/pf/config/cached.pm line 683.
I have not been able to get rid of this error even after a PF service restart.
This error continues even after a reboot of the box.
Nothing shows up in packetfence.log or admin_error_log.
Any assistance is greatly appreciated.
