I believe the status of each node (device) is stored in the MySQL backend.
If you mean how does PF use iptables to permit traffic from registered
systems, for inline mode, I believe it uses the mark functionality of
ipset. You can see how marks are used in the iptables rules in
iptables.conf.
I don't use inline mode, so someone please smack me if I am wrong.
