Fetching the up to date file from the github repo: wget https://raw.githubusercontent.com/inverse-inc/packetfence/devel/html/captive-portal/templates/guest.html
Come to think of it, this looks like an older template carried over after an upgrade. Make a backup copy of the old file and replace it with the one above. That will probably fix it.
