I'm using Pendragon Forms to do this, though not with quite so many Pilots

It interfaces directly with MS Access, whch of course, can front-end a SQL
Server database.

Per-seat costs run from $45 for singles to $30 for 20+ and I'm sure you
could come to some arrangement with pendragon if you're looking at buying
100,000 licenses.
