Since you're working on the second edition (which I'm looking forward to), I
should report a couple of minor issues:

1) Page 235, line 1: after "MemHandleUnlock (headerH)", shouldn't there be a
"DmReleaseResource (headerH)"?

2) page 237, the paragraph after the listing is clearly a humourous editing
mistake.  "unique idea" should be "unique id"
