Richard,

I've had the same problem.  The whole icon family stuff is a flakey.

I've gotten it to work by creating an icon family with one 1-bit icon and 
one 8-bit icon.  What seems to cause the problem is pasting a color image 
into the 1-bit icon, but I'm not sure what makes it work and what 
doesn't.  Now that I've gotten it working, I stay away from that color icon!
