We played around with the launchable bit as well, but OS 3.5 broke the
mechanism, as far as we can tell.
In our case, the document database itself wasn't changing, but if you
HotSynced a new copy of the application over the old one, the icons for
the documents would disappear like you describe. It's as if the link
between the application and the document was broken. Bummer, it was a
neat feature, but it's unusable in 3.5...
