4 bit (16 level) or 4 level (2 bit) grayscale?
First you need to switch the device into the greyscale mode. 16 level
bitmaps in resources and forms might not appear (given some posts
about what the OS supports or not) except on a IIIc, but you could use
color bitmaps.
Beyond that, you just normally need to set the screen pixel depth and
things should work.
