custom modal dialog

Hi,

I am interested in creating a custom modal dialog, that I can change the
text field and the button strings at run time.
FrmCustomResponseAlert does exactly this, but it supports only 3.5 feature
set.

any ideas on how to implement this with the 3.0 feature set?
