Try searching this list ( http://www.escribe.com/computing/pcpqa/index.html ) for "MenuCmdBarAddButton". There are previous answers to a similar question. One recommendation is to open the resource database using the dmModeLeaveOpen flag.
I don't think I've yet heard a good answer as to how to avoid resource ID conflicts with other notification command bar applications and/or with the foreground application.
