Dead files

It looks to me like exec_save.[ch] and exec_start.c are all unused.  I  
deleted local copies of them, and removed them from MANIFEST, and  
tried running a brand new build with --jitcapable and "make fulltest"  
and all ran cleanly.

Do any platforms use any of these?  If not, let's throw 'em away.
