There are places that hadn't been changed.
1. VTABLE visit function. (But I saw the pmc_freeze_cleanup had changed to
use VISIT_PMC_ATTR macro.)
2. a special place in VTABLE instantiate funtion.(Per chromatic, he'd like
to ask allison for her thoughts about such a change. see
