I've used phpIP before and it works reasonably well. http://www.phpip.net
The downside is that the latest version is from 2006 and I can only imagine the vulnerabilities it must have. Might make an interesting attack target, now that I think about it... Anyhow, with some careful configuration (mod_security, PHPIDS, etc) it might be ok. Just really consider what risks putting it on your network might bring.
