Hi Chris,
SayPad from
http://inspiredcode.net will do the trick. It's not exactly fast, but it 
allows you to denote chapters, and it runs on Sapi 5.
DSpeech I can't comment on, I haven't used it much. I use Kurzweil to do 
my stuff, but that doesn't help you. There are quite a few out there, 
though. A google search will turn up quite a few results, then it's a 
matter of preference.
