I use it with JAWS to manage my library and my portable MP3 players. I also use it to subscribe to my podcasts. I'm a pretty basic user though, and I don't know anything about iTunes or smart play lists.
I would say it's usable but not elegant. You have to do a lot of tabbing and use the F6 key to get around. It takes some trial and error to know what to do when. The on line features are also mostly inaccessible.
