I suggest sticking with simple options like "pca -l" and look for the "R" column.

"showrev -p" or "patchadd -p", listed patches have been applied.
4. What are the best options to use PCA in live server?
That depends on your needs. Stating from your questions you haven't worked at all with Solaris yet. Patching can be tricky and when you intend to work on a production system I recommend you ask a colleague for help to avoid problems. Please see that you get the proper training first.
