.. and that:

I have "debug=1" in my /etc/pca-proxy.conf file on my PCA proxy, but for
some reason it isn't writing a debug log in /var/tmp/.

.. strongly point at pca-proxy.cgi not reading the /etc/pca-proxy.conf file. Simple things first: Are the permissions set in a way, that the user running CGI scripts (or the web server) can read the config file?

BTW, you can remove this setting from pca-proxy.conf:

  threads=5

It has no effect on the proxy anyway (if multiple clients access the proxy, multiple CGI scripts will be spawned), and just adds more complexity to the case.
