Check to make sure the Startup Items is the correct one.  Does it 
have the custom ICON?  Drag it to the desktop, does the ICON revert 
to a standard folder ICON?  Drag it back to the System Folder, does 
the custom ICON reappear?

Is that System Folder the System Folder you are booting from?  Do you 
have more than one harddisk on your system?  Could your startup disk 
have more than one System Folder?
