Sounds like a 52xx/62xx motherboard.
I just pulled the board out of my LC630 which has been upgraded with a 5200
board and its almost identicle to your description except for the com slot
label. Mine just says "Communication slot".
Also I just noticed the CPU a 603e does not have a heatsink on it.
