Try replacing your telephone RJ-11 cable and then check the ports on the modem for 
misaligned pins
or movement (bad solder joint). I've replaced a couple of GV modems that became 
unreliable. 
Secondly toss TCP prefs file in the trash (after noting your correct settings) and 
re-enter the
settings after a restart.
