Aside from the battery systems can lose time by any application that
repeatedly plays .wav files or if sound applications are constantly being used.

Microsoft have acknowledged that there is a problem with some standard
drivers released in Windows 98 Second Edition. See

If the PC loses time only when the system is turned off, the problem is
almost always the battery. If the time loss is most serious when the system
is on, check the Device Manager for conflicts or disabled devices. (The
clock is interrupt-driven, and conflicting devices or software can impair
its operation.)

Next, systematically disable background software and utilities in the System
Tray, but leave Systray and Explorer running. If software isn't the problem,
try systematically removing optional hardware devices.

If you have determined that it's not the battery and not a program or a
conflicting deivice then it could indicate a more serious problem such as
the motherboard. You can replace the motherboard, adjust the time manually
or install a time syncronizer program like TimeServer.
