Thanks for the thoughtful replies - last night I got to grips with mrpeach's midifile and noteout and was able to get things working, it looks as though it will do the kinds of things I am after.
I am now using midifile to read and play the midi file, driving it with a metro, and tweaking its output on the way to noteout. I haven't tried controllers yet, but quite possibly they will work in a similar manner. So I guess that [midiout] is broken? For the record, seq-test.pd and seq-help.pd are in the pd-extended distribution, and both don't work for me (both use [midiout]. Thanks all for the suggestions & advice!
