The "correct" method is to use the Acrobat Distiller PPD which can be downloaded from Adobe.
 
After creating an Acrobat Distiller printer, you need to put a copy of the Distiller PPD in the Pagemaker RSRC folder.

I have been producing PDF in Pagemaker 7.0 WinXp w/ Distiller 5.0 and printing with Acrobat 5.0 on a HP 2500 Color LaserJet.
 
As it turns out, the "typical" driver installation selection set up this system to use the HP 2500 PCL6 driver.
After calling HP and getting help setting up the HP 2500 PS (postscript) driver manually, my PDF's are printing full size.
 
The PCL6 driver will not print the PDF's full size, no matter what, while the PS driver will print them full size using the driver's default settings.
