Why do you want to add a link to the start menu? I think this is the wrong model for launching PDF files. Do you have word documents in your start menu? PDF is a document format not an executable program. If you want to give your users more than launching from disk then tell them how to add your ebook to the Adobe Reader 'My Bookshelf' These are just my opinions, you might have entirely valid reasons for putting documents in the start menu, if so please tell us your reasons.
