Make sure you choose RADIO BUTTON from the drop down menu in your form choices. At the 
moment it is probably a CHECK BOX. Then name both fields be the SAME name, but under 
options they must have DIFFERENT EXPORT OPTIONS. Ie Yes or No. Then choose your deisgn 
from the drop down list (star, square, cross etc.) 

Then when you click on one, the other will go off.
