What version of Acrobat? Which exactly command are you using?
Adobe Tech Support said it was "expected behavior" and suggested I look for a third-party tool to preserver NAME attributes.
As far as I know, there aren't any currently shipping tools that will convert HTML documents with form fields to PDF files with form fields.
