[PDF-Forms] Extractig Data From Form Fields

Does anyone know how I can accomplish the following:   I have Adobe Acrobat 6.0 Pro.  I have a pdf file that I created with tons of form fields in it that are basically text fields with drop downs with number 1-10 in them.   Anyway, I want to email this form to 5000 people and have just the data that's in each of the form fields to be accumulated into one document. I have seen it done before on someone's web server.  However, I want to know how to do it.  I am willing to pay for it.  but I have yet to find someone who can tell me exactly what I need.   Thanks for any help.
