Enfocus Pitstop offers a tool called Action lists, that enables the user to find and change PDF objects into other PDF objects - perhaps you could send them an example of the 'before' PDF and then, manually built and 'example' PDF of what you want - then open it in Illustrator - and modify it further, then save as PDF. Perhaps they can build you a custom Action list for a small fee. If I recall - unless things have changed with the current version of the Acrobat SDK - some things like "layers" are available in Illustrator, and can be written into a PDF by Illustrator, but are not available to PDF Plug-in developers.
