This is done using the Form tool that is built into Acrobat.

If you have the full version of Acrobat, go to Help index and look under
"form fields".  This will explain how to use the form tool.

I would also recommend purchasing any one of the number of books that are
available on Acrobat, just to use as a reference.  You can find a large
selection at any of the larger book chain stores, or on line.  Acrobat has a
lot of functionality that most users don't even know about.
