Quite Software's Quite Imposing or Quite Imposing Plus
will do this for you. Regardless of the document's source
(working only with the PDF version), this plugin will do the
conversion the way you want (two A4 pages reduced to
A5), and will also automatically paginate the document.
This means that if you're doing (for instance) a 20 page
document, the first page produced will contain page 1
and page 20, the next 2 and 19 etc.
