This, in my opinion, is a "serious" design flaw.

If I prepare a document, and want to add a stamp such as "confidential,"
then there's a 100% chance that I want to see it on any documents printed
from the PDF. As the creator, I don't want this to be an option for the user
to turn off.

If this issue is critical for you, you can buy a third-party tool to stamp
the document. You can also include your own stamp in your original document.

Adobe support told me I had to print with comments but that makes some kind
of odd document that deletes everything after the stamp.

I couldn't get it to work, either.

I make a pdf document from several files and then click the Confidential
stamp and place this stamp on the cover. When my client physically prints
the document the stamp doesn't show. It shows as being on the document on
her screen but won't print on paper.

How are stamps handled in 6.0 that's different than in 4.0?
