why does part of this form's text go missing?

I am attempting to use PdfBox to print the attached PDF. When it prints, the 
acrofields' data values appear, as do the lines of the form. But none of the 
text of the form appears. I've created other PDFs that use the same fonts, and 
print. I'm using PrintPDF command-line utility. any insights would be greatly 
appreciated.
