One method of tracking changes is to submit
the users name/datetime to a database via a
form submittal - 

Is this a voluntary or required step ??

This submittal would insert a new record into
the database yet not be enabled with an
update function, protecting the previous entries.

This will also require an automated query when
the PDF document opens to load the tracking log
from the database...

This is very doable using the FDFtoolkit.
