In Acrobat 5 and earlier, this would be done using a plugin. In Acrobat 6, it can be a plugin OR a JavaScript.
When i create a plug-in. I can be able to add the button all the time, the reader is loaded.
You simply have your plugin add/remove the toolbar button on the fly - no big deal.
The LARGER issue is that in order to have a plugin that will work in Reader, you need a license from Adobe Systems. See http://partners.adobe.com/asn/tech/pdf/ikla.jsp.
