Brent,
  This sounds like you are talking about a 3270 terminal attached to an 
OS/390 or Z/OS system?

If that's the case I would do the following:
        Pick one of the many available 3270 Screen Scraping tools to capture 
the data displayed by the terminal.
        Use the captured data to build an FDF or XFDF file. ( both are simply 
structured text files. )
        Import the FDF or XFDF into the Acrobat form file.

This can be done relatively easily in either a desktop or server 
environment.  What are your requirements?
