The rebate form is slightly different to the one on the Calumet website. The calumet one (which is a mail-in rebate directly to colorvision) states that it the rebate only allowable on the spyder if purchased at a retail price of $199 or higher. Other than that, it appears to identical, even down to the Offer #.
