Nobody?  Not quite.  Leica still makes a fully manual camera.  In 
fact, they've recently come out with the MP which is described by some 
as the epitome of fully manual camera design. Using one is an 
experience every photographer should have, if only once :-)

There are numerous fully manual cameras in the medium format arena as 
well.  The 'blad comes quickly to mind, as does the base Mamiya 645 
(been using one of those for a while myself - sweet piece of gear).  
Believe the Rolleiflex is still being produced :-)

Real photographers don't need AF, don't need no steenkin' batteries, 
either.
