I designed it and had it made by this company: <http://www.srbfilm.co.uk/> The components used are a simple Canon AF-to-M42 brass adapter, drilled out to EOS spec for mounting on the lens, and a small round plate fabricated from scratch to maintain the register distance. Longer mounting screws are also needed. All details on that web page I gave you.
