My first thought is that it could be the infamous Fuji pepper-grain effect. My Minolta Multi Pro sees this clearly even at 3200ppi... but I think yours looks a little different.
The solution I'm trying is to order a Scanhancer, but that is only made for the Multi Pro. The Minolta 5400 has it built-in - can you gain access to one of these for a comparative scan of the same film?. Have you tried the Despeckle filter?
If you're not scanning Fuji film you can ignore this :)
