Looks like the infamous pepper grain effect. It's apparently quite common with hi-res scans and varies greatly with the type of film. I found Provia 100F to be quite bad, and Kodak E100SW scanned very well.

As for general solutions, I don't know. A Scanhancer solved it for me but that's only available for two Minolta scanners - it's built into the 5400 and is available from its inventor for the Multi Pro (the scanner he developed it for).
