Not sure about Wintel. Pantone ColorVision supports dual screens on the Mac platform, but not as well as I'd like it to (the PreCal program only supports the main screen). That may have been fixed in an update: I haven't checked for a while.
Having said that, even ColorSync only really supports one monitor. The second screen has its profile applied but the system colour manages the whole desktop as if it's the main screen. I presume that things would get extremely difficult as you start moving your windows across both screens.
I only use my main screen for colour-critical work anyway: the other just holds toolboxes and stuff.
As for apps, I don't know. Maybe Photoshop can tell which monitor an image window is on, and can adjust itself appropriately. Maybe not.
