I've tried IE 6.0 and Firefox 1.5. The drop-down list box isn't
populated with anything; I can't therefore select which gallery I want,
and the form won't submit.

Which browsers and versions are you referring to?
The form was made in 2002, and should be working in all browsers since then. I
know Mozilla/Firefox may be a bit more picky than the rest of the pack, but I
haven't tested it with the form.
Internet Explorer should work fine, as should Netscape, Opera and Safari.
