A piece of rubber between my thumb and the cap usually does the trick. A piece of double sided tape for the somewhat stubborn ones. Unfortunately I run into ones that have come loose and been firmly glued back in place, for those a pair of pliers with perfectly square mating jaws is the only answer I've found. Even then I scratch a few. And yes, it is clockwise to loosen.
