Just an okay retouching job . You can tell that it's primarily a perspective adjustment. The paper she's holding has been stretched as well. Also, her right hand is actually broader in the "thin" version, since it's in a horizontal position, but was stretched vertically. A top-notch retoucher would have pasted in the thin hands and the unaltered piece of paper. I do think the crushing of the blacks and the saturation increase is a plus.
